Job summary

VitalCaring Group is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for field-based home health. You will provide direct patient care under RN supervision, implement care plans, and deliver skilled nursing services in patients’ homes, building trusted relationships and supporting recovery.

You will monitor conditions, educate families, and document care in the EMR, while maintaining infection control and collaborating with an interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.

Responsibilities

• Provide direct patient care under the supervision of the RN.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
