Job summary

VitalCaring is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for home health, delivering hands-on patient care in patients' homes and supporting the clinical team. Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate from an accredited practical/vocational nursing program.
• Current CPR certification and clean clinical record.
• Minimum one year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care under RN supervision.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately within the EMR.
• Communicate with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences and ongoing training.
• Support coordinated care with consistent team communication.
