Job summary

St. Elizabeth Home Care is seeking a compassionate LPN/LVN to deliver direct patient care at home per approved treatment plans and clinical protocols. You will collaborate with the RN Case Manager and care team to ensure high-quality patient outcomes.

The role requires strong communication, documentation accuracy, and the ability to work within a multidisciplinary team. CPR/BLS certification and a valid state license are required, with home care experience preferred.

Qualifications

• Graduate of an accredited practical nursing program.
• Current license to practice as an LPN/LVN in the respective state.
• CPR/BLS certification required and maintained.

Responsibilities

• Collaborate with RN Case Manager and health team to ensure care per treatment plan.
• Conduct patient visits per physician treatment plan and regulatory guidelines.
• Make skilled observations and document care provided accurately.
• Report changes in patient condition to supervisors and attending physician promptly.
• Provide patient education to patients and caregivers on care plans and health management.
• Demonstrate sensitivity to patients and families’ needs and cultural differences.
• Participate in case conferences, in-services or orientations as required.
• Complete and submit all required documentation accurately and promptly.
