St. Elizabeth Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to join our team and deliver direct patient care in home settings under approved treatment plans.

You will collaborate with the RN Case Manager, conduct visits, document care accurately, and educate patients and families while adhering to clinical guidelines and regulatory requirements. Mileage reimbursement is provided.

Competenze

• Graduate from an accredited practical nursing program and hold a valid LPN/LVN license.
• Current CPR or BLS certification maintained during employment.
• Minimum one year nursing experience; home care or hospital preferred.
• Excellent clinical, written and verbal communication skills.
• Ability to work in a multi-disciplinary team and build relationships.
• Valid driver’s license and auto liability insurance as per policy.
• Experience with EHR systems such as HomeCare HomeBase is a plus.

Mansioni

• Collaborate with RN Case Manager to ensure patient care per plan.
• Conduct patient visits per physician treatment plan and regulations.
• Make skilled observations and document care accurately.
• Report changes to supervisors and attending physician promptly.
• Educate patients and caregivers on care plans and health management.
• Demonstrate sensitivity to patients' needs and cultural differences.
• Participate in case conferences and trainings.
• Complete and submit all required documentation promptly.
