Job summary

Interim HealthCare of Queen Creek & Gilbert is seeking a Home Health LPN to provide patient-centered care in patients' homes across East Valley communities, with per diem scheduling.

You will exercise autonomy, perform assessments, develop and implement plans of care, and educate patients and families while delivering high-quality nursing in home settings.

Qualifications

• Arizona LPN license active and in good standing.
• Minimum of 1 year LPN Home Health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

What You'll Do

• Perform patient assessments and contribute to care planning


• Develop and update individualized plans of care aligned with physician orders and patient goals


• Deliver skilled nursing services in the home setting


• Educate patients and caregivers on conditions, treatments, medications, and self-care


• Provide preventive, restorative, and rehabilitative nursing interventions


• Monitor progress, identify changes in condition, and coordinate appropriate follow-up


• Maintain timely, accurate clinical documentation


• Collaborate with the interdisciplinary team to support continuity of care

What You Bring

• Graduate of an accredited nursing program with an active Arizona LPN license


• Minimum of 1 year of LPN experience in Home Health preferred


• Home health, wound care, or case management experience preferred


• Current CPR certification


• Valid driver's license, auto insurance, and reliable transportation


• Strong clinical judgment, communication, organization, and documentation skills
