Job summary

Interim HealthCare of Queen Creek & Gilbert is seeking a Home Health LPN to deliver skilled nursing in patients' homes across Queen Creek and surrounding East Valley communities. You will perform assessments, develop plans of care, educate families, and coordinate care with the interdisciplinary team.

We offer per diem scheduling with competitive pay ($48.00–$55.00 per hour) based on experience, flexibility, and strong clinical autonomy to influence patient outcomes in a home setting.

Qualifications

• Active Arizona LPN license.
• Minimum 1 year of LPN experience in Home Health.
• CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform patient assessments and contribute to care planning.
• Develop and update individualized plans of care aligned with physician orders and patient goals.
• Deliver skilled nursing services in the home setting.
• Educate patients and caregivers on conditions, treatments, medications, and self-care.
• Provide preventive, restorative, and rehabilitative nursing interventions.
• Monitor progress, identify changes in condition, and coordinate appropriate follow-up.
• Maintain timely, accurate clinical documentation.
• Collaborate with the interdisciplinary team to support continuity of care.

What You'll Do

What You Bring

• Graduate of an accredited nursing program with an active Arizona LPN license


• Minimum of 1 year of LPN experience in Home Health preferred


• Home health, wound care, or case management experience preferred


• Current CPR certification


• Valid driver's license, auto insurance, and reliable transportation


• Strong clinical judgment, communication, organization, and documentation skills
