VitalCaring Group is seeking a Licensed Practical Nurse (LPN/LVN) for field-based home health care. You will provide direct patient care under RN supervision and help implement individualized care plans in patients’ homes.

You’ll deliver skilled nursing procedures, monitor patient status, and reinforce education to support recovery and independence in a familiar environment.

Qualifikationen

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
