Job summary

VitalCaring Group is seeking a licensed Practical/Vocational Nurse (LPN/LVN) for home health, delivering direct patient care under RN supervision in patients' homes. You will implement care plans, provide skilled nursing services, and educate patients and families.

The role emphasizes consistent, compassionate care with a flexible field-based schedule and collaboration with an interdisciplinary team to improve patient outcomes.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.
