Job summary

VitalCaring is seeking a Licensed Practical Nurse (LPN/LVN) for home health in the Orlando area. In this field-based role, you’ll deliver direct patient care under the supervision of an RN, implement care plans, and support recovery and independence in patients’ homes.

You’ll perform skilled nursing duties, monitor conditions, educate patients and caregivers, maintain EMR documentation, and collaborate with an interdisciplinary team.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while reporting changes in condition.
• Basic proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.

You are a key part of the care team - delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.

What You’ll Do

• Provide direct patient care in accordance with the state Nurse Practice Act

• Deliver skilled nursing services, including treatments, medication support, and wound care

What You Bring
