Job summary

All Heart Home Care in Murfreesboro, TN is seeking compassionate Licensed Practical Nurses (LPNs) to join our expanding home health team. We offer Full-Time, Part-Time, and PRN positions to deliver high-quality, patient-centered care in patients’ homes with a supportive interdisciplinary team.

Responsibilities include skilled nursing under RN supervision, medication administration, wound care, education for patients and caregivers, accurate documentation, and travel within the service area to

Qualifications

• Current LPN license in Tennessee with active credentials.
• CPR certification required.
• Valid driver’s license with reliable transportation.
• Strong communication, organizational, and clinical skills.
• Ability to work independently and manage patient visits.
• Home health experience preferred but not required.

Responsibilities

• Provide skilled nursing care in patients' homes under RN supervision.
• Monitor patient conditions and report changes to the team.
• Administer medications and routine nursing care as ordered.
• Assist with wound care, disease management, and education.
• Educate patients and caregivers on medications and home care.
• Document patient visits and care in accordance with policies.
• Maintain communication with patients, families, and physicians.
• Promote comfort, safety, and independence in the home.
• Travel throughout the service area to provide care.
