UnitedHealth Group is seeking a Licensed Practical Nurse to provide direct home health care to acutely and chronically ill patients in Tennessee, under RN supervision. You will document visits, coordinate care, and support families with empathy and professionalism.

Formación

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance; access to a dependable vehicle or public transportation.
• Current CPR certification or ability to obtain within 90 days of hire.
• Ability to work flexible hours to meet identified client needs.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.

Responsabilidades

• Provides high quality clinical nursing services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits in the EMR per regulatory and payer requirements.
• Participates in care coordination by reporting needs and changes, attending case conferences, and communicating with the patient, family, physician, and team.
• Functioning effectively in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
