Job summary

Hand N Hand Home Healthcare, Inc. in Farragut, Tennessee is looking for a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing services and ensure high-quality care. This position offers both full-time and part-time flexible scheduling.

The ideal candidate must be a graduate of an accredited LPN program and possess an active LPN license in Tennessee. You'll work within a supportive environment, collaborating with RN Case Managers and care teams, to make a meaningful impact in clients’ lives.

Qualifications

• Minimum 1 year of nursing experience preferred; home health experience a plus.

Responsibilities

• Perform skilled nursing assessments and implement care plans under RN direction.
• Administer medications and treatments safely and accurately.
• Complete clinical documentation and report changes in client condition promptly.
• Communicate effectively with clients, families, and care teams to ensure continuity of care.
