Empower Home Health is seeking compassionate Licensed Practical Nurses (LPNs) to work one-on-one with patients in home environments, providing education and skilled nursing to foster independence. Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of current CPR certification and valid Driver’s license.
• Ability to work and flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in planning and coordination of total patient care with RN and physician's plan of care.
• Accurately report and record patient's condition and care, including signs and symptoms of change.
• Prepare clinical/progress notes and summaries.
• Assist patient with activities of daily living while teaching self-care techniques.
• Follow universal precautions and all emergency procedures.
