Job summary

Cooperative Home Care, Inc. is seeking a compassionate Licensed Practical Nurse to provide skilled nursing care to patients in their homes on an intermittent basis.

You will work under the supervision of an RN and follow physician orders and the patient’s plan of care. The role requires nursing licensure, reliable performance, and strong communication skills to educate patients and caregivers, document care, and participate in care conferences and quality assurance activities.

Qualifications

• Graduate of a state-approved Practical Nursing program.
• Minimum of one year of nursing experience preferred.
• Current CPR certification.
• Ability to work independently and exercise sound judgment.
• Strong communication, organization, and time-management skills.
• Commitment to compassionate, patient-centered care.

Responsibilities

• Provide skilled nursing services and treatments per physician orders and patient plan, monitoring changes and reporting to supervising RN and physician.
• Educate patients and caregivers on care needs, treatments, and self-care techniques.
• Maintain accurate patient documentation and participate in care conferences and quality activities as assigned.
• Maintain confidentiality, clean bag technique, and a safe work environment per policies.
• Demonstrate competency, report incidents, maintain attendance, and participate in on-call duties including nights, weekends, and holidays.
