Qualifications

• Active PA LPN license or multistate license required.
• TB test and BLS/CPR certification required (in person).
• Valid driver's license essential for home visits.
• Experience or willingness to train in G-tube, trach, or vent care.

Responsibilities

• Collaborate with RNs and clinical leadership to implement plans of care.
