Job summary

VitalCaring Group in Tampa, Florida is hiring an Home Health Licensed Practical Nurse (LPN/LVN). You will provide direct patient care in patients' homes under RN supervision, implement care plans, and support independent patient recovery.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act
• Implement and support the plan of care developed by the RN
• Deliver skilled nursing services, including treatments, medication support, and wound care
• Monitor patient condition and report changes to the RN and care team
• Educate patients and caregivers on self-care, treatments, and disease management
• Document patient care accurately and timely within the EMR
• Communicate effectively with physicians, nurses, and interdisciplinary team members
