Job summary

VitalCaring Group seeks a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for field-based Home Health in Florida. You will provide direct patient care under RN supervision, supporting care plans and helping patients recover and stay independent in their own homes.

This role offers one-on-one visits, flexible scheduling, and a collaborative clinical team. Candidates should have an active LPN/LVN license, CPR certification, and reliable transportation to serve an assigned

Qualifications

Active LPN/LVN license in the state of practice.
Graduate of an accredited practical/vocational nursing program.
Current CPR certification.
Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsibilities

Provide direct patient care in accordance with the state Nurse Practice Act
Implement and support the plan of care developed by the RN
Deliver skilled nursing services, including treatments, medication support, and wound care
Monitor patient condition and report changes to the RN and care team
Educate patients and caregivers on self‑care, treatments, and disease management
Maintain infection control and proper use of equipment and supplies
Document patient care accurately and timely within the EMR
Communicate effectively with physicians, nurses, and interdisciplinary team members
Participate in case conferences, team meetings, and ongoing training
Support coordinated care by maintaining consistent communication across the care team
