Job summary

Interim HealthCare of Central MT is hiring a Home Health Licensed Practical Nurse to deliver high-quality in-home nursing care across Montana. The role emphasizes flexible scheduling, autonomy, and a focus on patient-centered outcomes.

You will assess patients, manage vitals, and collaborate with RNs and CNAs to execute the plan of care while educating families on medications, safety, and discharge planning.

Qualifications

• Must hold Montana LPN license and CPR certification.
• 1 year of LPN experience, preferably in home health.
• Excellent communication and compassionate bedside manner.

Responsibilities

• Provide home-based nursing care to patients after hospital discharge.
• Collaborate with RNs, CNAs, and aides to deliver quality care.
• Take vitals, assess and document progress; report changes to RN supervisor.
• Assist with plan of care and teach self-care to patients.
• Educate family about plan of care, meds, and home safety.
• Coordinate care and discharge planning with interdisciplinary team.
