Job summary

LaPlace LA PCS, a part of LHC Group, is seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders. You will document visits, coordinate care, and work under supervision to ensure high-quality patient outcomes.

The role requires an active LA LPN license, CPR, and reliable transportation. Strong communication and the ability to handle diverse home environments are essential.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within the scope of practice and in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record per requirements.
• Participates in the agency's care coordination, communicating changes in condition to the team.
