Job summary

UnitedHealth Group's Baton Rouge LA PCS, part of LHC Group within the Optum family, seeks an LPN to provide direct home nursing care under physician orders. You will document visits and coordinate patient care in the service area, ensuring high-quality outcomes for acutely and chronically ill patients in their homes.

Requirements include LA LPN licensure, CPR, a valid driver's license, and at least 1 year of LPN experience.

Qualifications

• Current and unrestricted LPN licensure in LA.
• CPR certification current and in good standing.
• Valid driver's license and access to a vehicle or reliable transport.
• Ability to work in home settings regardless of client demographics.

Responsibilities

• Provide high quality clinical nursing services per physician plan of care.
• Document patient visits thoroughly in the electronic medical record.
• Coordinate care and communicate with patients, families, physicians and care team.
• Adhere to infection control procedures and regulatory requirements.
