Job summary

Boone Memorial Home Care is seeking an experienced Licensed Practical Nurse to deliver skilled nursing care in patients’ homes within the LHC Group network. This role supports home health services, hospice and community-based care under the Optum family, emphasizing compassionate, patient-centered support.

The LPN will document visits, assist with care coordination, and adhere to infection control and regulatory requirements while maintaining professionalism and patient safety.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license or reliable transportation.
• Flexibility to meet client needs and schedule.

Responsibilities

• Provides high-quality clinical nursing services within scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician and care team.
• Function in diverse home settings with respect and professionalism.
