Job summary

Amedisys is hiring a licensed practical nurse to join our home health care team in Virginia. This is a full-time day role delivering in-home care to patients who feel most comfortable at home.

Responsibilities include verifying plans of care with the RN, collaborating with physicians, teaching patients and caregivers self-care, taking vital signs, and performing medication reconciliations during visits. Orientation pay applies with competitive benefits.

Qualifications

• One year of clinical LPN experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g., foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
