Job summary

Qualifications

One year of clinical LPN experience.
Current LPN license, state(s) you are assigned to work.
Current CPR certification.
Valid driver''s license, reliable transportation and liability insurance.

Responsibilities

Verify plan of care with the RN prior to each visit.
Collaborate with physician and RN to ensure patient needs are met; notify if patient condition changes.
Teach patients and caregivers self-care techniques.
Check vital signs.
Review medications and perform medication reconciliation at each visit.
Perform specialized procedures ordered by physician (e.g. foley catheter change).
Prepare equipment and materials for treatments using aseptic technique.
Complete documentation timely and accurately.
