Job summary

Amedisys is seeking a licensed practical nurse for full-time day shifts to provide compassionate home health care. You will verify plans of care with the RN, work closely with physicians and RNs, and support patients and families in self-care and medication safety.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
