Emblem Home Health in Chandler, AZ is seeking a Licensed Practical Nurse (LPN) for a full-time home health role. You will deliver direct patient care under RN supervision, following care plans and ensuring comfort and safety in patients' homes.

This position emphasizes growth through mentorship, ongoing education, and collaboration with a caring team across the East Valley, including Mesa, Chandler, and Gilbert.

Qualifications

• Current LPN license in the State of Arizona.
• 3 years of nursing experience preferred (Home Health or MedSurg a plus).
• Current CPR certification and valid driver's license may be required.

Responsabilités

• Deliver compassionate, direct patient care in home health per state regulations.
• Implement care plans developed by the RN and document patient progress.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure high-quality service.
