Job summary

Amedisys Home Health is hiring a licensed practical nurse for full-time day shift to provide compassionate home health care. You’ll verify plans of care with the RN before each visit and collaborate with physicians to meet patient needs.

You will educate patients and caregivers, monitor vital signs, reconcile medications, and assist with procedures ordered by physicians, ensuring timely documentation and safe aseptic technique.

Qualifications

• One year of clinical LPN experience.
• Current LPN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
