Job summary

Mass General Brigham in Massachusetts seeks a licensed practical nurse (LPN) to provide professional nursing care under the direction of an RN Case Manager and Clinical Manager. You will perform skilled nursing visits in patients' homes, clinics, and community settings.

Requirements include MA LPN licensure, BLS certification, and a valid driver’s license with reliable transportation. Home care or hospice experience is preferred. On-call coverage and weekend/evening/holiday rotations apply.

Qualifications

• Graduate of an approved, accredited school of professional nursing.
• Current licensure to practice as an LPN in Massachusetts.
• Current BLS certification required.
• Home care or hospice experience preferred.
• Knowledge of homecare regulatory and reimbursement requirements.

Responsibilities

• Provide professional nursing care under the direction of a RN Case Manager and/ or Clinical Manager.
• Perform skilled nursing visits for patients in homes, clinics, and community settings.
• Follow care plans and report changes to the Case Manager/Clinical Manager.
• Participate in on-call coverage and weekend/evening/holiday rotations.
• Communicate promptly and accurately per agency policy.
