Job summary

Millennium Physician Group is seeking a Licensed Practical Nurse to join our Home Health Team in Lee County, FL. You will provide skilled nursing care under RN supervision, perform essential procedures, and support patients and families in their homes.

The role requires current Florida LPN licensure, at least 6 months of clinical experience, and strong communication, professionalism and reliability. Travel to patient homes is required, with a focus on high-quality outcomes.

Qualifications

• Current and valid LPN license in Florida.
• Minimum of 6 months of clinical LPN experience in a hospital or skilled nursing facility.
• Excellent communication, interpersonal and critical thinking abilities.
• Professionalism with reliability and quality care commitment.
• Reliable transportation to visit patients at their homes.

Responsibilities

• Provide high-quality nursing care including medications, wound care and vital signs.
• Conduct comprehensive clinical assessments and develop care plans.
• Administer medications and educate patients on regimens.
• Implement prescribed treatments, including IV therapy and catheter care.
• Educate patients and families on disease management and self-care.
• Collaborate with physicians, therapists and other healthcare providers.
• Maintain accurate and timely patient care documentation.
• Adhere to infection control guidelines and provide emotional support.
