Job summary

Choice Health At Home is seeking a Home Health Field LPN in Federal Way, WA for a full-time role delivering in-home nursing care. The LVN/LPN administers medications, conducts wound care, and supports chronic disease management in a home setting.

Under RN supervision, you will assist in planning care, educate patients and families, and coordinate with physicians to implement individualized care plans. A valid LVN license, driver’s license, CPR, and HomeCare HomeBase experience are preferred.

Qualifications

• Current LVN License
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification
• HomeCare HomeBase experience preferred

Responsibilities

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
