Job summary

Choice Health At Home is seeking a Home Health LVN/LPN for full-time work in the Athens, TX home health branch. The LVN/LPN will provide high-quality nursing care in patients' homes, assess health status, administer medications, and support wound care under RN supervision.

Responsibilities include planning with the RN per the plan of care, applying infection control, ensuring continuity of care, and communicating with patients and families to educate on treatment protocols and health maintenance.

Qualifications

• Current LVN License is required.
• Valid Driver’s License and Auto Insurance are required.
• CPR Certification is required.
• HomeCare HomeBase experience preferred.

Responsibilities

• Identify patient needs under the direction of the RN and document findings.
• Plan and implement care in conjunction with the RN per the POC.
• Apply infection control concepts to prevent contamination and transmission.
• Ensure continuity of care through quality patient care delivery.
• Communicate effectively with patient, family, and health care team.
• Monitor cases to meet third party payor requirements and policies.
