Job summary

Choice Health At Home is seeking a Home Health LVN/LPN to provide in-home nursing care across the Denver Metro area. Under RN supervision, you assess patients, administer medications, and implement the plan of care to support independence and recovery.

This full-time role requires an active LVN license, a valid driver's license and insurance, CPR certification, and reliable transportation. HomeCare HomeBase experience is preferred, with Monday–Friday coverage and on-call rotation.

Qualifications

• Current LVN license
• CPR certification
• Valid driver’s license and auto insurance
• HomeCare HomeBase experience preferred

Responsibilities

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation and evaluations.
• Plan and implement care in conjunction with the RN, in accordance with the POC.
• Apply infection control concepts and standard precautions in coordinating/performing patient care activities.
• Ensure continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals.
