Job summary

AccentCare is seeking a Licensed Practical Nurse (LPN) for home health in Renton, WA. The role involves providing direct patient care under RN supervision across pediatric to geriatric patients, with a Monday–Friday schedule and full-time status.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of operation.
• One year of LVN/LPN work experience.
• Must possess and maintain CPR certification.
• Must be a licensed driver and able to travel to all locations.

Responsibilities

• Performs technical nursing functions and direct patient care under RN supervision.
• Promotes and maintains optimal client care under RN supervision.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities to case manager and home health team.
• Administers prescribed medications and treatments and assesses medicines for interactions or adverse effects.
• Re-evaluates client's nursing needs and reports to Case Manager.
• Counsels and instructs the client and family in nursing needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner per agency policy.
• Documentation on patient charts correlates with the care plan.
• Communicates with other home care team members frequently.
• Assists in coordinating services provided to client.
