Job summary

National Church Residences is seeking a Full-Time Home Health LPN to provide and direct nursing care across southern Ohio. The role involves supervising daily rounds, implementing care plans, and coordinating with physicians and families to ensure quality resident care.

The LPN will follow established policies, support staff, monitor medications, and uphold HIPAA and safety standards while delivering exemplary patient service during Monday–Friday daytime shifts.

Qualifications

• Graduate of an accredited school of nursing (LPN/LVN).
• Current LPN license in good standing.
• 1–2 years of home health or clinical experience preferred.
• Must be able to read, write and speak English.
• Knowledge of infection control practices and HIPAA compliance.

Responsibilities

• Supervise, provide and direct nursing care following facility policies.
• Observe, report, and document resident conditions.
• Prepare and administer medications per policy; manage MAR/TAR.
• Staff supervision and assignment of nursing assistants.
• Ensure resident safety, nutrition, and comfort; communicate with families and physicians.
• Maintain accurate documentation and confidentiality.
