Boone Memorial Home Care is seeking a Licensed Practical Nurse to provide compassionate, quality care in patients' homes. You will deliver skilled nursing services within regulatory guidelines and the plan of care, document visits, and collaborate with the care team to coordinate patient needs.

Formación

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Home care experience is a plus.
• Ability to manage multiple tasks and work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet client needs.

Responsabilidades

• Provides high quality clinical services within the scope of practice and plan of care, following infection control procedures.
• Documents patient visits thoroughly in the electronic medical record as required.
• Participates in care coordination, communicates with patient, family, physician and care team.
