Job summary

Mountaineer Home Care, part of LHC Group, seeks a Licensed Practical Nurse in Home Health to provide quality patient care in the patient’s home and to coordinate with the health care team. You will document visits, participate in care coordination, and follow infection control procedures to ensure safe, effective care.

Required: LPN licensure in state, driver’s license, and ability to travel to patient homes.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.
• Home care experience.
• Ability to manage multiple tasks simultaneously.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws.
• Documents patient visits thoroughly and completely per regulatory and payer requirements.
• Actively participates in the agency's care coordination process including timely reporting of patients' needs and changes in condition.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
