Elite Home Health, part of LHC Group and the Optum family, is hiring a Licensed Practical Nurse to provide high-quality in-home health services. The role involves direct patient care in home settings, documentation, and care coordination with the health team.

You will travel to patients’ homes, perform skilled nursing tasks, and ensure infection control and regulatory compliance while delivering compassionate, patient-centered care.

Formación

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilidades

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, communicates with patient, family, physician, and team.
• Functionally adapt to diverse home situations and client needs.
