Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance.
• CPR certification preferred or ability to complete within 90 days of hire.
• Ability to manage multiple tasks simultaneously; work independently.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes in condition.
• Functions effectively in diverse home settings and with clients of all backgrounds.
