Job summary

UnitedHealth Group, via Mederi Caretenders Viera (part of LHC Group), is hiring a Licensed Practical Nurse in Home Health to deliver quality patient care in patients’ homes and support the care team.

Primary duties include providing skilled nursing services per the plan of care, documenting visits in the electronic medical record, and coordinating with the patient, family, physician and other clinicians. LPN licensure and a valid driver's license are required.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license with vehicle insurance or access to dependable transportation.
• CPR certification preferred or able to complete within 90 days of hire.
• Ability to work independently and manage multiple tasks.
• Strong communication, writing, and organizational skills.

Responsibilities

• Provides high quality clinical services within the scope of practice per state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in the agency's care coordination process and communicates with patient, family, physician, and team.
• Functions effectively in home settings regardless of client demographics.
