Job summary

Munroe Regional HomeCare, part of the LHC Group and Optum family, is seeking a Licensed Practical Nurse (LPN) in Ocala, Florida. You will provide high-quality clinical services in patients' homes, document care in the EMR, and coordinate with the health care team per the plan of care.

Qualified candidates must hold an active LPN license in Florida, have a valid driver's license with reliable transportation, and possess strong communication and organizational skills.

Qualifications

• Current and unrestricted LPN licensure in state of practice
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• CPR certification or ability to complete within 90 days of hire
• Ability to manage multiple tasks simultaneously
• Strong communication, writing, and organizational skills
• Ability to work flexible hours as required to meet identified client needs

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements
• Participates in care coordination including reporting needs and changes in condition
• Functioning in diverse home settings and with diverse client populations
