Job summary

UnitedHealth Group in Cincinnati seeks a Licensed Practical Nurse for home health to provide high quality patient care in patients' homes, following the plan of care and infection control standards. Primary duties include completing visits and documenting thoroughly in the EMR, coordinating care with the care team, and communicating clearly with patients, families, and physicians.

Must hold a current LPN license in the state, CPR, a valid driver's license, and be able to work flexible hours.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly and completely per regulatory and payer requirements in the EMR.
• Actively participates in the agency's care coordination process and communicates effectively with patient, family, physician, and care team.
