Job summary

Home Health of Wilson, part of LHC Group, is hiring a Licensed Practical Nurse to deliver high-quality care in patients' homes. You will perform skilled nursing tasks, document visits in the EMR, and collaborate with the care team to update plans of care.

The role requires current LPN licensure in the state, a valid driver's license, and CPR certification or the ability to obtain it within 90 days. Home care experience and strong communication skills are preferred.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance or access to transport.
• CPR certification or ability to complete within 90 days (preferred).
• Home care experience (preferred).
• Ability to manage multiple tasks simultaneously.
• Ability to work independently.
• Strong communication, writing, and organizational skills.
• Willingness to work flexible hours to meet client needs.

Responsibilities

• Provide quality clinical services within scope of practice and plan of care.
• Document visits thoroughly in EMR per regulations and payer requirements.
• Participate in care coordination, report changes in condition, communicate with team.
