Job summary

UnitedHealth Group is seeking a dedicated Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients' homes under RN supervision.

You will verify plans of care, collaborate with physicians and RNs, teach self-care techniques to patients and caregivers, and perform essential nursing tasks during visits, ensuring accurate documentation and ongoing quality of care.

Qualifications

• 1+ year of LPN/LVN experience.
• Current LPN license, state-specific.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
