Job summary

UnitedHealth Group—a leading healthcare company in the United States—seeks a Home Health LPN/LVN to provide skilled nursing in patients' homes under RN supervision. You will verify plans of care, coordinate with physicians and RNs, and educate patients and caregivers on self-care throughout visits.

The role requires an active LPN license in the state(s) of assignment, reliable transportation, CPR certification, and at least 1 year of LPN/LVN experience.

Qualifications

• Current LPN license, state(s) licensed to work
• Valid driver’s license and reliable transportation
• Current CPR certification
• 1+ years of LPN/LVN experience

Responsibilities

• Verify plan of care with the RN prior to each visit
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review medications and perform medication reconciliation at each visit
• Perform physician-ordered procedures (e.g., Foley catheter changes)
• Prepare equipment and materials using aseptic technique
• Complete documentation timely and accurately

Skills

Current LPN license
