Job summary

Ohioans Home Healthcare seeks a dedicated Licensed Practical Nurse to provide in-home patient care across Fulton, Henry, Defiance and Williams Counties under RN supervision near Toledo. You will assess, plan and implement care, educate patients and families on diseases, procedures and medications, and handle wound care and device care.

Qualifications

• Current LPN state licensure.
• Three years of nursing experience.
• CPR certification.
• Ability to travel for in-home patient visits.
• Exceptional time-management skills.
• Excellent interpersonal and customer service skills.
• Home healthcare experience preferred.

Responsibilities

• Assess, plan and implement patient care under RN supervision.
• Educate patients and families on diseases, procedures and medications.
• Address patient needs including wound care, trachs, G/J tubes, Foley catheters and medication administration.
• Maintain electronic records via iPad and portable printers.
• Build long-term one-on-one relationships with patients and support continuity of care.
