Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a licensed practical nurse (LPN) to join our Home Health team in Frederick, MD.

Provide 1:1 patient care per individualized plan, administer medications and treatments, and document care and communications with families and the care team. This full-time role offers a supportive environment, team collaboration, and opportunities for professional growth.

Qualifications

• LPN/LPT/LVN degree with current unrestricted registration and license in state of practice
• Valid driver's license with acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home health care experience preferred

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting progress and outcomes for established goals
• Coordinating or ordering supplies, medications, and feedings as needed to ensure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician
