Job summary

Liberty Home Care in Thomasville, NC is seeking an experienced Home Health LPN to provide direct patient care in home settings and coordinate with RNs and the care team. You will document thoroughly and ensure appropriate plans of care are followed.

Requirements include a current NC LPN license, CPR certification, and about one year of clinical nursing experience, preferably in acute care. The role requires strong communication and independent organization of work.

Qualifications

• Successful completion of nursing education from an approved school.
• Current LPN licensure in the state where the branch is located (North Carolina).
• One year of clinical nursing experience, preferably in an acute care setting.
• Ability to independently organize work and provide effective home-based care.
• Effective communication skills (oral and written).

Responsibilities

• Provides direct patient care using a variety of skills and coordinating services.
• Completes documentation as required by policy, procedure and regulation.
• Follows the plan of care developed by the Registered Nurse.
• Coordinates homecare services with the RN, Patient Care Coordinator and other staff.
• Accepts and transcribes physician's orders as directed.
• Forwards clinical, progress and summary notes to the RN for coordination.
• Attends and participates in patient care conferences.

HOME HEALTH NURSE – LPN

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse – Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
