Job summary

Liberty Health Greensboro, NC is seeking an experienced Home Health LPN to provide direct patient care in home settings under the supervision of a Registered Nurse. You will follow the plan of care, document thoroughly, and coordinate with care teams to support patient independence and recovery.

Requirements include completion of an approved nursing program, current LPN license in North Carolina, CPR certification, and strong communication skills.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in North Carolina.
• At least 1 year of clinical nursing experience preferred.
• Ability to organize work and provide home health care independently.
• Excellent communication skills; CPR certification.

Responsibilities

• Provide direct patient care in home settings per plan of care.

• Transmit notes to RN for coordination; participate in care conferences.

HOME HEALTH NURSE - LPN

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse - Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
