Liberty Health Fayetteville, NC seeks an experienced HOME HEALTH LPN to provide direct patient care in home settings, following the plan of care and coordinating with RNs and care teams.

You will document thoroughly, communicate effectively, and participate in care conferences, ensuring high-quality home health services for adults and homebound patients. Valid NC LPN licensure and CPR are required.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state where the branch is located and one year of clinical nursing experience (preferably acute care).
• Ability to independently organize work and provide effective home healthcare services.
• Effective communication skills (oral and written).
• CPR certification required.

Responsabilités

• Provides direct patient care in home settings and coordinates services with the RN and care team.
• Completes documentation in accordance with policy, procedures, and regulations.
• Follows the plan of care developed by the RN for each patient.
• Coordinates homecare services with physicians, care coordinators, and other agency staff to optimize patient care.
• Accepts and transcribes physician orders as directed and forwards notes to the RN for coordination.
• Attends and participates in patient care conferences.

HOME HEALTH NURSE – LPN

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse – Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
