Job summary

Liberty Home Care in Dunn, NC is seeking an experienced LPN to provide compassionate, in-home nursing care to patients recovering from illness, surgery or injury. The role focuses on delivering direct patient care and coordinating with the Registered Nurse and care team.

You will document care, follow the plan of care, schedule visits, and participate in care conferences. Requires nursing diploma, NC LPN license, CPR, and strong communication skills.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in North Carolina and one year of clinical nursing experience.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the RN and document thoroughly, accurately, and completely.
• Effective communication skills (oral and written).
• CPR certification.

Job Description

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse – Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
