Job summary

Emblem Home Health is seeking an LPN to provide hands-on patient care in patients’ homes under RN supervision. This full-time role requires a licensed practical nurse with Arizona licensure, CPR certification, and reliable transportation to travel across Gilbert, Chandler, Mesa, and Tempe.

You will implement care plans, monitor patient conditions, and collaborate with a multidisciplinary team to ensure safe, quality care and positive patient outcomes.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the State of Arizona.
• 3 years of nursing experience preferred (Home Health or MedSurg a plus).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high‑quality service.
• Participate in professional development and team meetings.

This is a full-time position offering a Monday-Friday schedule and requires travel across the Gilbert, Chandler, Mesa, Tempe
